Field Nurse Practitioner / Physician Assistant

Field Nurse Practitioner – up to $165,000/year

HealthCare Support is seeking a Field Nurse Practitioner to join a rapidly growing healthcare provider serving patients in Casa Grande, AZ. This is a unique opportunity for an NP who enjoys building relationships with patients, working independently in the community, and collaborating with an interdisciplinary care team to improve outcomes for patients with complex medical and social needs. You'll manage a panel of medically complex CKD patients, spend more time building relationships, make home visits, collaborate with an interdisciplinary care team, and have the ability to truly impact outcomes while maintaining a Monday-Friday schedule. Enjoy a dynamic mix of in-home visits and telehealth opportunities, all while making a real difference in underserved populations.

Compensation: $148,500 - $165,000 annually

• Base Salary: $135,000 - $150,000
• Annual Bonus: Up to 10%
  
  

Schedule:

• Monday – Friday, 8:00 AM – 5:00 PM
• No routine weekends or overnight shifts.
• Limited after-hours calls
  
  

Daily Responsibilities:

• Providing primary care and comprehensive assessments in patients' homes.
• Managing patients with CKD and multiple chronic conditions.
• Completing approximately 5–6 patient visits per day / 20 visits per week

Required Qualifications

• Active Nurse Practitioner (NP) license in Arizona
• 2+ years of experience as a practicing NP in Primary Care
